Position also requires travel to Mesa, Flagstaff, WA and NV.
As the Palliative Care Clinical Director, you will be responsible for the operational leadership, supervision, provision, and documentation of the palliative care interdisciplinary team in accordance to Federal and State regulations. You will provide clinical leadership in conjunction with and under the auspices of the palliative care service medical director to achieve consistent high-quality patient care, outcomes, and satisfaction. Coordinates and completes assigned duties to support effectively the immediate and long-range objectives of the company. Enhances profitability and sustainability of the agency. Recruits, retains and manages an effective and efficient clinical team. Facilitates and participates in interdisciplinary team meetings, in-services, committees, quality improvement, and other meetings. Reviews and recommends changes in policies, procedures, and forms. You will also manage the clinical interdisciplinary team including Palliative Care NPs, RNs, Social Workers, Chaplains, and office staff. Assists the medical director as needed.
Primary Responsibilities:
- Lead day-to-day palliative care operations, coordinating referrals and care management workflows to support timely, high-quality service delivery
- Coordinate patient care communication and interdisciplinary collaboration (including scheduling/facilitating IDG meetings) to ensure continuity of care
- Oversee clinical documentation and required reporting (referrals, visit notes, transfers/discharges, events), ensuring accuracy and compliance
- Coordinate and support the palliative plan of care in partnership with the Medical Director/NPs; collaborate with patients/families, providers, and the interdisciplinary team to achieve goals
- Conduct patient visits as needed; assess patient needs, document condition, and escalate unaddressed needs to the physician/NP promptly
- Provide team leadership, staffing support, onboarding/orientation, training/in-services, and performance management; participate in QAPI and drive follow-up on action plans
- Support operational/financial stewardship, including scheduling/resource optimization, coding/billing oversight, and partnering on budget and performance goals

Required Qualifications:
- Active RN license in state of practice
- 2+ years clinical experience
- Experience collaborating in an interdisciplinary team environment
- Valid driver's license, valid vehicle insurance, and access to dependable transportation (vehicle or public transportation)
Preferred Qualifications:
- Current CPR certification or ability to obtain with in 90 days of employment
- Hospice or palliative care certification
- Hospice or palliative care experience
- Clinical experience in the home environment
- Leadership experience (finance, budget, community resources, quality outcomes, customer service, growth, and team building)
- Knowledge of palliative care and ability to communicate professionally with referral sources, healthcare partners, and colleagues regarding disease trajectory, clinical assessment, and appropriate interventions (including physical, psychosocial, and spiritual needs)
- Demonstrated independent problem-solving, sound judgment, and ability to manage office staff, clinical professionals, and providers
- Demonstrated ability to balance workload and prioritize multiple tasks while maintaining a positive, service-oriented leadership approach
- Ability to shift focus throughout the workday and handle sensitive issues with thoughtful prioritization of action and outcomes
Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). No matter where or when you begin a career with us, you'll find a far-reaching choice of benefits and incentives. The salary for this role will range from $91,700 to $163,700 annually based on full-time employment. We comply with all minimum wage laws as applicable.
