Street Medicine and Mobile Behavioral Health ARNP (FNP or FNP/PMHNP)

Providence St. Peter Hospital is seeking a mission driven, compassionate Psychiatric and Family Nurse Practitioner to join the established Street Medicine and Mobile Behavioral Health Clinic serving the unhoused population in Olympia. This is a full-time position working Monday through Friday, offering meaningful work paired with a predictable and sustainable schedule.
This role is embedded within a highly collaborative team that includes RNs, social workers, a clinic manager, and behavioral health partners dedicated to integrated, compassionate care. This is not a desk job. Care is delivered through a mobile clinic, encampments, shelters, community partner sites, and during transitions from hospital to community-based care.
The Role:
You will provide a hybrid of primary care and behavioral health services for a highly complex and underserved population. Practice occurs at the intersection of primary care, psychiatry, and addiction medicine in a flexible, field-based environment.
- Provide mobile acute care, chronic disease management, psychiatric screening and diagnosis, and medication management
- Conduct universal mental health and substance use screening across visits
- Manage substance use disorders, including medication management and injectable buprenorphine when appropriate
- Exercise independent clinical judgment to determine when patients require emergency intervention versus field-based care
- Support proactive outreach and structured follow up for patients with complex medical and psychiatric needs
- Coordinate hospital discharges, medication access, referrals, and continuity of care
- Collaborate closely with community partners, case managers, and behavioral health programs to streamline transitions of care
- Practice trauma informed, relationship centered care addressing medical, social, and psychological determinants of health
Safety is a priority. All street outreach is conducted in teams of at least two with established safety protocols and communication check ins.
Required Qualifications:
- Nurse Practitioner with both Family Nurse Practitioner and Psychiatric Mental Health Nurse Practitioner certification
- Experience caring for complex, underserved, or unhoused populations strongly preferred
- Comfort providing both primary care and behavioral health services in mobile or non-traditional settings
- Ability to function independently while working within a collaborative team-based model
- Interest in population-based outreach, integrated care, and innovative practice models
Compensation and Benefits:
- Compensation is between $154,097 and $207,701 per year
- The compensation amounts listed may include productivity or quality/service incentives, call pay, extra shift incentives, as applicable to the position. In addition, providers may have CME benefits available to them
