IMMEDIATE OPENINGS FOR SOCIAL SERVICE OUTREACH SPECIALIST FOR DOWNTOWN CHICAGO

Streetplus Company, LLC is looking for experienced social service outreach specialists to circulate throughout downtown Chicago, focused on identifying and building positive relations with unhoused and unsheltered persons in need of services. The position helps people cope with challenges they face in their life and refers them to available resources structured to deal with their challenges.

The position will concentrate on engaging individuals that are unhoused and unsheltered with the goal of referring them to agencies, organizations, and programs structured to provide assistance. The position will also identify agencies, organizations, and programs and schedule regular meetings to stay connected and updated on available resources.

RESPONSIBILITIES

§ Circulate throughout the downtown and engage unhoused and unsheltered persons

§ Maintain communication with clients engaged while addressing their challenges

§ Coordinate with agencies, organizations, and programs structured to assist unhoused and unsheltered persons.

§ Ensure compliance with regulatory requirements

§ Maintain case management file on each client

§ Track engagements, referrals, and outcomes using Streetplus software and HMIS

§ Complete Vulnerability Assessment on each client and create list of scores for each client

§ Create “Top 10” list bi-weekly, using results from the Vulnerability Assessment.

§ Communicate with other social service outreach teams dedicated to assisting unhoused and unsheltered persons.

§ Address Quality-of-Life ordinance violations.

§ Attend meetings with service providers and City of Chicago departments with the purpose of sharing data and outcomes.

§ Develop and maintain positive business relationships with agencies, organizations and departments focused on providing assistance to unhoused and unsheltered persons.

COMPENSATION AND BENEFITS

§ Starting hourly pay rate of $26.00 per hour

§ Health Insurance (80% of premium paid by Streetplus) – after ninety days

§ Dental Insurance (100% of premium paid by Streetplus – after ninety days

Requirements

§ Bachelor’s Degree in social service-related discipline

§ Valid and current LCSW or LMSW license

§ Three years of experience working with unhoused and unsheltered persons

§ Demonstrated experience providing street-level services

§ Professional written and verbal communication and interpersonal skills

§ Experience with Microsoft Office products

§ Ability to be mobile for extended periods of time (walking and standing)

§ Ability to work flexible schedule
