Description
Summary/objective
The Director of Social Services will provide support in all aspects of social services for all Daily Living Centers (DLC) locations. The Director of Social Services will work closely with the VP of Operations.
Essential functions

Obtain and maintain working knowledge regarding adult day pay sources to include DDS, DHS state funds, Advantage Medicaid waiver, VA, LTC insurance
Attend trainings and receive updates for DDS, DHS, Advantage Medicaid waiver, and VA
Assist participants and caregivers with financial applications and renewals
Track pay source authorizations and expirations of participants and lead communication with caregivers and update Transition Log weekly
Oversees new participant intake process for all locations of DLC (with the assistance of the Office Manager)
o Answer inquiry calls, schedule and lead center tours, send out intake packets to prospects
o Enter inquiries into CADcare and communicate with Center Directors regarding status of inquiry process
o Assist in pay source identification process and communication with caregivers
o Receive and verify completion of intake packets and enter intake info into CADcare
o Complete intake checklist in timely manner
Maintains professional contact and rapport with DHS, Advantage case managers, DDS case managers and VA to assist Participants and Caregivers in making application and reporting enrollments and discharges
Create and submit monthly reports to President & CEO to include inquiries, intakes, discharges, and referral information
Follow up on pay source denials in timely manner
Maintain positive professional relationships with community partners for participant and volunteer referrals
Oversee Daily Living Centers Volunteer Program
Weekly Check-Ins with Center Directors to grow census
Leads DLC internal United Way employee campaign
Attend community booths and health fairs to promote awareness of Daily Living Centers
Lead caregiver/participant survey process
Coordinates and leads Caregiver Support program (monthly support groups, community resources, caregiver communications and updates)
Maintain positive and strong rapport with caregivers and participants at each center
Requirements
Required education and experience
Bachelor’s degree or Associate’s degree in social work, gerontology, or related field
Intermediate knowledge of Microsoft Office Suite
Strong time management skills a must
Strong work ethic and communication skills
Flexible schedule for after hours events
